In this meeting, we'll discuss past support experiences, current goals, strengths and needs, and available services and funding options at Strata and Threshold Community Partners. 

Intake meetings can be done virtually or in person at our office. After completing the form, we'll reach out to you to confirm scheduling.

Intake meetings and new client processing are a $60 flat rate. Missed appointments or cancellations made less than 24 hours in advance will incur a charge at the intake rate.
